Responsibilities for Behavioral Health Case Managers: 
  • Provides Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill clients in an outpatient facility and in the community.
  • Establishes therapeutic relationships, assesses complex problems, selects problem-solving interventions, and helps clients to function effectively.
  • In conjunction with a supervisor and team psychiatrist, assesses the client’s mental health and basic needs, determines the appropriate level of care and support, determines the availability of resources, and develops an Individual Recovery Plan.
  • Coordinates access to the recommended medical treatment services, vocational assistance, housing assistance, and other targeted services.
  • Monitors the treatment progress of clients, reassesses client status and the effectiveness of interventions, revises the Individual Recovery Plan, and advocates for clients.
  • Coordinates, conducts or participates in assessments; provides crisis assistance services; delegates recovery plan tasks to appropriate service providers; and monitors overall service delivery.
Requirements for Behavioral Health Case Managers:  
  • Bachelor's degree in Psychology, Social Work, Human Services, Education, or another field related to the treatment of mentally ill persons or a minimum high school diploma plus 3 years experience in mental health. 
  • Experience in social services and behavioral health preferred but not required.
  • Position requires frequent driving of own personal vehicle.  A valid driver’s license, vehicle, and insurable driving record are required.
