Job Description
Responsible for providing support services to ACT (Assertive Community Treatment) staff and clients. Maintains ACT charts and data. Triages client phone calls, assists with client engagement and housing stabilization, and follows up on client’s success with housing.

Position Responsibilities:
  • Willing to share insight from past lived experiences to help clients in their own recovery.
  • Assists care managers in helping program participants obtain housing and complete tasks related to housing stabilization and maintenance in accordance with ACT fidelity standards.
  • Assists with client engagement, including in person visits and coordination with other service providers and housing supports.
  • Completes appropriate documentation, daily on provided services.
  • Provides on-site assistance with service delivery and coordination.
  • Provides peer support services to ACT clients as needed. This may be individual or group support services.
Position Requirements:
  • Individual must be a mental health consumer and/or have previous experience with homelessness or substance abuse issues required.
  • Knowledge of substance abuse system, mental health system, and homeless community-based services strongly desired.
  • Individual must have demonstrated ability to respectfully motivate other consumers to act on their own behalf and to participate in activities.
  • High school graduate or GED required.
  • Good communication and organizational skills.
  • Basic computer knowledge.
  • Position may require frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record may be required.
