Job Description
NEW PROGRAM - Greater Cincinnati Behavioral Health Services (GCB) is expanding service delivery to individuals with substance use disorder experiencing homelessness in Hamilton County.  This new program will provide an integrated service delivery model which includes housing, mental health and substance abuse treatment, and employment services. 

Provides Care Management Services to individuals experiencing homelessness and substance abuse using a Housing First / Harm Reduction Model.  Participates in outreach and engagement, establishes therapeutic relationships, assesses complex problems, selects problem-solving interventions, and helps clients meet basic needs to function effectively.   Coordinates, conducts or participates in assessments, provides access to recommended treatment services (behavioral health, medical), employment services, housing assistance, and other targeted supportive services.  Provides crisis assistance; connects clients with basic community services, resources, and opportunities; trains clients in the use of those resources and monitors overall service delivery.  At all stages of client intervention, the AOD Care Manager is responsible to oversee access to needed services, allocate, monitor, and evaluate services and/or fiscal resources/benefits such as Medicaid or Medicare applications, housing, banking, etc.

Position Responsibilities:
  • Completes face-to-face contacts with individuals who are homeless throughout Hamilton County to identify potential clients for program (includes making contact with homeless individuals on the streets, parks, soup kitchens, shelters, health care facilities, jails, etc.).
  • Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in life areas including:  living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, ac­tivities of daily living, vocational/educational, social supports, leisure and recreation.
  • Provides support services to assist clients in obtaining housing through the Coordinated Entry process and maintaining housing through the Critical Time Intervention model.
  • Attempts to engage the client in identifying and expressing his/her own needs through assertive outreach efforts, offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis.
  • Assesses the client’s basic needs and readiness for substance abuse treatment and availability of resource
  • Develops, implements and monitors a personal service plan and Individual Recovery Plan in conjunction with the client and treatment team.
  • Gains knowledge of the following: homelessness, substance abuse and mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client’s use of services, and applicable costs of use, and the opportunities for alternative resources.
  • Provides coordination and assistance in accessing service for crisis stabilization and resolu­tion, and collaborates in identifying treatment needs when client is receiving services from other parts of the crisis response system. 
Position Requirements:
  • Bachelor's degree in one of the specified fields of study plus two years of specialized education, training, or experience in one of the specified fields of study. 
  • Master's degree in mental health, social work, counseling or other health related field, preferred. 
  • Experience working with individuals who are homeless and/or struggling with substance abuse.
  • Credentialing as a licensed Social Worker, Counselor or Chemical Dependency Counselor, preferred. 
  • Successful applicant must pass pre-employment drug test.
  • The position requires frequent driving of the employee’s personal vehicle.
