Provides Care Management Services to homeless individuals with chronic substance abuse problems using a Housing First / Harm Reduction Model. Participates in outreach and engagement, establishes therapeutic relationships, assesses complex problems, selects problem-solving interventions, and helps clients to function effectively. Coordinates, conducts or participates in assessments, access to the recommended medical treatment services, vocational assistance, housing assistance, and other targeted services and monitors overall service delivery.
AOD Care Manager - Full time position at the Jimmy Heath House
Position Responsibilities:
Makes face-to-face contacts with individuals who are homeless throughout Hamilton County to identify potential clients for program (includes making contact with homeless persons on the streets, parks, soup kitchens, shelters, health care facilities, jails, etc.).
Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in areas to include: living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, vocational/educational, social supports, leisure and recreation.
Coordinates access to the recommended medical treatment services, vocational assistance, housing assistance, and other targeted services.
By assertive outreach, attempts to engage the client in identifying and expressing his/her own needs, offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis.
Gains knowledge of the following: homeless, substance abuse and mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client’s use of services, and applicable costs of use, and the opportunities for alternative resources.
Coordinates and participates in the team's development of a personal service plan which will include services and assistance necessary to achieve specific objectives, as well as identifies the availability, intensity, responsibility and duration required of each service.
Provides coordination and assistance in accessing service for crisis stabilization and resolution, and collaborates in identifying treatment needs when client is receiving services from other parts of the crisis response system.
Assists the client to achieve goals and maximize independence and productivity through support, teaching, and training in the use of personal and community resources.
Works collaboratively with team including other care managers, supervisor, clerical support worker, and team psychiatrist to provide service to clients. Ongoing communication is necessary with all team members.
Reviews and monitors services and activities and documents progress according to organization procedures for client records, program documents and special reports.
Position Requirements:
Bachelor's degree in one of the specified fields of study plus two years of specialized education, training, or experience in one of the specified fields of study.
Master's degree in mental health, social work, counseling or other health related field preferred.
Experience working with individuals who are homeless and/or chronic substance abusers.
Credentialing as a licensed Social Work, Counselor or Chemical Dependency Counselor (CDCA) preferred.
Successful applicant must pass pre-employment drug test.
