Job Description
Provides Care Management Services to individuals with severe mental illness through outreach and engagement, establishes therapeutic relationships, resolves complex problems using appropriate problem-solving interventions, and helps clients to function effectively.  Identifies each client’s strengths and limitations and the types of service, financial, and institutional resources available.  Collects information about a client’s medical, voca­tional, social, educational, housing, financial, transportation, and other basic needs.  In conjunction with a Lead Care Manager, reviews the client’s basic needs and readiness for treatment, as well as the availability of resources.  Implements and monitors “My Action Plan”.  Coordinates access to the recommended medical treatment services, vocational assistance, housing assistance, and other targeted services. At all stages of client intervention, the Associate CM has authority to access, monitor, and evaluate services and/or fiscal resources such as Medicaid or Medicare application, housing, etc.


Position Responsibilities:
  • Makes face-to-face contacts with individuals who are mentally ill throughout Hamilton County.
  • Strives to develop and maintain therapeutic relationships with clients. 
  • Initiates, coordinates, and monitors services necessary to meet each client's needs in areas to include:  living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, ac­tivities of daily living, vocational/educational, social supports, leisure and recreation. 
  • Through assertive outreach, attempts to engage the client in identifying and expressing his/her own needs and offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis.
  • In conjunction with a Lead Care Manager and Supervisor, reviews the client’s basic needs and readiness for treatment, availability of resources, and may participate in the development of the treatment plan.  
  • Gains knowledge of the following: homelessness, substance abuse and mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client’s use of services, and the opportunities for alternative resources.
  • Implements the personal service plan which will include services and assistance necessary to achieve specific objectives, as well as identifies the availability, intensity, responsibility and duration required of each service. 
  • Assists the client to achieve goals and maximize independence and productivity through support, teaching, and training in the use of personal and community resources. 

Position Requirements:
  • Associate's degree in human services field of study plus two years of specialized education, training, or experience required.  Bachelor’s degree preferred.
  • Excellent oral and written communication skills, good documentation, time management, coordination and collaboration skills.
  • Ability to work with clients in a variety of environments during crisis situations, to mobilize community resources, and to be self-directed.
  • Basic computer knowledge. 
  • Position requires the use of a personal mobile phone. 
  • Position requires frequent driving of own personal vehicle.  A valid driver’s license, vehicle, and insurable driving record are required.
