Job Description
Provide case management (community psychiatric support treatment) with a focus on assisting clients to develop resilience factors, to the co-occurring mentally ill and developmentally disabled (MI/DD) population in the community in order to help them live as independently as possible in the community.

***This position will be based in both Clermont and Hamilton Co.

Position Responsibilities:
  • Provides case management with a focus on assisting clients to develop resilience factors, to the co-occurring mentally ill and developmentally disabled (MI/DD) population in the community in order to help them live as independently as possible in the community.
  • Provides services accessing community resources, advocacy, support and collaborative follow-up with identified referral sources through face-to-face and telephone service as needed to assist individuals and families with meeting established goals.
  • Ability to create, implement and evaluate treatment plans according to individual client needs.
  • Performs a variety of case oriented duties, including case documentation, review of clinical assessment information to assist with client evaluation.
  • Attends/coordinates Trauma Informed Biographical Timeline meetings, Trauma informed supervisory meetings, and other program or unit meetings as required, participating in discussions regarding clinical, program, and administrative issues.
  • Submission of treatment plans, clinical and billing documentation according to the agency’s paperwork policies.
  • Works collaboratively with team including, therapist, DD worker, and community providers to promote resiliency within the client.  Ongoing com­munication is necessary with all team members.
  • Conscientiously observes the confidential nature of the agency's services.
  • Responsible for maintaining professional licensure requirements.
Position Requirements:
  • BA in Psychology, Social Work, or a related Human Services field or a minimum high school diploma plus 3 years experience in mental health. 
  • Position requires frequent driving of own personal vehicle.  A valid driver’s license, vehicle, and insurable driving record are required.
  • Position requires working with indiduals in both Hamilton and Clermont Counties
