Job Description

The ideal candidate will have an active LSW/LPC and/or CDCA or someone who is willing to achieve the CDCA certification within 3 months!

ACT Team Members serve as members of a multidisciplinary team and provides services as both a specialist and a generalist to the all clients on the team. Clients on the ACT team are those who meet ACT Diagnostic criteria of severe mentally illness, whose functioning is significantly impaired by their mental illness and who have not been successful with traditional care management services. The specialist role of the Substance Abuse Specialist provides individual and group outpatient substance abuse treatment services to clients who are dually diagnosed severely and severely mentally ill. 

Position Responsibilities:

  • Provides services as both a specialist and a generalist to all ACT team clients
  • Generalist duties of all ACT Team Members focus on establishing therapeutic relationships, assessing complex problems, and helping clients to function effectively in the community.
  • Specialist role of Substance Abuse Specialist involves provides individual and group outpatient substance use disorder treatment services to persons with severe mental illness who are dually diagnosed with substance use disorder. 
  • Interventions will be consistent with the client’s assessed Stage of Change. 
  • Provides education and training to team regarding Integrated Dual Disorder Treatment (IDDT) principles and leads staging exercises on the in conjunction with the ACT Team Leader. 
  • Monitors the treatment progress of clients, reassesses client stage of treatment and the efficacy of interventions. 
  • Requires night, weekend and Holiday shifts. Valid driver’s license, vehicle, and insurable driving record are required.
  Position Requirements: 
  • Bachelor’s or Master’s degree in a Human Services field or High School diploma and a minimum of three years experience in Behavioral Health or Human Services related field.
  • CDCA required within 3 months of hire OR currently holds an active LSW/LPC.
  • Position requires use of personal mobile phone.  Position requires frequent driving of own vehicle.  Valid driver’s license, vehicle and insurable driving record are required.
