Job Description
Are you a registered nurse and want to help individuals in the community? This position is eligible for our full benefits package and hours are Monday-Friday 8:30am-5:00pm. 


Position Responsibilities:
  • Provide nursing services (primarily med/somatic) on site and in the community to clients with a psychiatric nursing focus.
  • Administer medications and injections and provide medical/health screening and minor physical health duties (dressing changes/blood pressure screening/monitoring glucose levels/etc.).
  • Provide psychiatric nursing assessment of individual clients as it relates to medication complications.
  • Assess effectiveness of psychotropic medication therapy and provide education to clients regarding topics such as medications, side effects, and drug interactions.
  • May be assigned to initiate, coordinate, and monitor evaluations necessary to identify patient needs in areas to include:  living arrangements, financial concerns, physical/psychiatric health, activities of daily living, vocational/educational, social supports, leisure and recreation.
  • By assertive outreach, attempts to engage the patient in identifying and expressing his/her own needs, offers services to assist in meeting these needs within job duties, and re-assesses needs and outcomes with the patient.
  • A primary focus will be relapse prevention/mental and physical health promotion; will work closely with the teams to identify/evaluate factors leading to relapse, will collaborate with team and other providers to develop and implement service interventions to ensure patient holistic health stability.
 
Position Requirements:
  • Licensed by the Ohio Board of Nursing as Registered Nurse required.
  • BSN strongly preferred.
  • Experience in working with persons who have a chronic mental illness preferred.
  • Excellent communication, documentation, and time management, skills.
  • Ability to be flexible with work schedule and assignments.
