Job Description
Become an integral part of Behavioral Health with GCB at the McMicken Health Clinic!  The Behavioral Health Consultant is a dependent social worker or counselor and working towards independently-licensed master’s level social worker or counselor (LISW/LPCC) who is embedded within GCB's partnering health centers. The primary focus of the BHC is to deliver brief holistic, action-oriented, evidence-based services to primary care patients, and immediate concise consultation to medical providers. An active member of the primary care team, the BHC assists with implementation of primary care provider treatment plans, and supports diverse patients in achieving improved health outcomes, accelerating the patient’s experience of care, and efficiency of the practice. BHC identifies, triages, and empowers patients to make healthy lifestyle choices by activating self-management strategies, and motivational techniques to prevent or manage chronic physical and behavioral health conditions. This position is at a local FQHC (Federally Qualified Health Center) named McMicken Integrated Clinic/ Cincinnati Health Network, which specifically serves individuals experiencing or within a year of experiencing homelessness. We also free license supervision hours and continuing education credits.

Full Time Position working in our Primary Care Health Centers!

Position Responsibilities:  
  • Assists primary health providers in recognizing and treating patients, understanding the connection between biological, psychological, and social health, and the ability to quickly collaborate with patient to formulate a plan of action toward well-being.
  • Performs targeted brief assessment and evaluation, including diagnostic impressions and functional status focused on presenting problem, engaging client in self-care and self-management strategies, lifestyle enhancements, and motivational techniques to prevent or manage chronic physical and behavioral health conditions.
  • Triages and refers patients to specialty mental health care when appropriate; formulation of behavioral interventions appropriate to the primary care setting, and assisting with the implementation of treatment plans.
  • Develops, teaches and/or provides oversight for group classes that promote education and skill-building to enhance psychological and physical health; may work in conjunction with medical providers to offer group classes.
  • Utilizes cultural competence to understand patient’s expression of psychological distress, cultural perspective on health and health problems, and seek resources as needed. Specifically needs to have knowledge and skill of working effectively with individuals experiencing homelessness.
  • Works as a primary care team member to develop specific clinical pathways or best practice programs for targeted patient groups for population health.

Position Requirements:
  • Master’s degree in social work or counseling and within 3 months of receiving independent license by the Ohio Social Worker Board (LISW/LPCC).
