Medical Assistant/Program Assistant position provides data management support services to the Clermont ACT team. The position collects and reports the project’s data. This includes information gained from client from face to face interactions or collection of data from team meetings, GCB reports and client charts. The information includes service data, outcome data, client survey data and health data preferably including vital signs. The Program Assistant manages and reports all required data through charting or entering data into spreadsheets or databases.
**Part time position working 20 hours/week.
Position Description:
Conducts initial and follow-up NOMS surveys.
Collects, manages and submits NOMS.
Collects, manages and evaluates all other service, performance and outcome data.
Provides administrative support for all data management activities.
Engages clients to successfully participate in housing, case management, SUD treatment, employment and other supportive services deemed beneficial to each client’s recovery.
Collects and records health data and reports data to Team Prescriber and RN.
Completes contact notes and other documentation on provided services within expectations.
Enters contact and demographic data in database.
Provides occasional transportation of clients in personal vehicle.
Position Requirements:
A minimum of a High school degree or equivalent required.
A medical Assistant certification is strongly preferred.
Knowledge of mental illness and/or substance use disorders strongly desired.
Experience with data collection including conducting personal interviews desired.
Position requires the use of a personal mobile phone. Position requires occasional driving of own vehicle. A valid driver’s license, vehicle, and insurable driving record are required.
