Job Description
GCBHS has an immediate opening for a Community Connections Specialist/Case Manager to provide short-term case management (community psychiatric support treatment) services our Child & Family Division and need assistance with medical, educational, and resources for daily living and self sufficiency. 

The ideal candidate will have a BA in Psychology, Social Work, or a related Human Services degree and experience working with individuals with behavioral health issues.

Position Responsibilities:
  • Works with therapist to assess and evaluate clients to identify and remove barriers through case management intervention that would prevent clients from completing treatment.
  • Provides case management, connections to community resources, other duties as required to individuals and their families who have been assessed and referred by the assigned therapist. 
  • Assists clients in accessing community resources, advocacy, support and collaborative follow-up with identified referral sources.  Provides face-to-face and telephone service as needed to assist individuals and families with meeting established goals.
  • Performs a variety of case oriented duties, including case documentation, review of clinical assessment information to assist with client evaluation.
  • Formulates and reviews recommended interventions, referrals, and treatment of clients with therapist and other designated parties as indicated.
  • Works closely with agency therapist and community resource providers and collaborators.
  • Works with clients for up to 3 months.  If further care is needed, the client is re-assessed for services.

Position Requirements:
  • BA in Psychology, Social Work, or a related Human Services field and experience in behavioral health.
  • Position requires frequent driving of own personal vehicle.  A valid driver’s license, vehicle, and insurable driving record are required.

Salary and Benefits
$34,521-$36,930 plus bonus
Pay Differential for MA Degree! We pay a starting rate of $34,521 for BA and $36,930 for MA. We also offer a $1,200 annual bonus (paid as $300 quarterly) in addition to our competitive salary.
