Job Description

Join our mission to help individuals with mental illness and addictions to lead healthy and productive lives.  GCBHS has an immediate opening for a Case Worker to help adults who are experiencing severe mental illness, substance use disorder, and homelessness in Cincinnati.

This position is part of a unique team which focuses on helping individuals who are experiencing homeless and assesses them for mental health needs.  The ideal candidate will have social service experience and experience working with individuals experiencing homelessness. 


Position Responsibilities:

  • Makes face-to-face contacts with individuals who are homeless throughout Hamilton County to identify potential clients for program (includes making contact with homeless persons on the streets, parks, soup kitchens, shelters, health care facilities, jails, etc.).
  • Assesses individuals who are homeless for mental health needs.
  • Makes multiple contacts as needed and provides basic need items to engage individuals who are experiencing homelessness and mental illness.
  • Interacts with agencies, service providers and other community contacts to solicit and receive referrals.
  • Develops psychosocial profile through interviews with individual and information collected from previous service providers.
  • Identifies and connects individuals served to services aimed at addressing mental health issues as well as other basic needs, including medical services and housing.
  • Prepares and submits all required reports of activity, maintain files and records of work activities to provide access to and retrieval of data.  Submits service documentation for outreach contacts by the next working day.
 
Position Requirements:  
  • Related experience working with persons experiencing homelessness, mental illness, and/or substance use disorder.
  • High School Diploma or Equivalent required.
  • Position requires frequent driving of the employee’s personal vehicle. Valid driver’s license, vehicle, and insurable driving record are required.
