This is a great starting point for a recent Social Work or Psychology graduate or an individual with Human Services experience.

We are looking for candidates with a HS Diploma and 3+ Years of related Mental Health/Human Services experience or recent graduates with a bachelor's degree in Psychology, Criminal Justice, Sociology, Social Work, or a related Human Services field.

Responsibilities for Transitional Support Specialist: 

• Perform intake functions and provide intensive Community Psychiatric Support Treatment (CPST) services and Therapeutic Behavioral Services (TBS) to clients with severe mental illness living in or transitioning to the community.  Services to include medication training, independent living skills training, interpersonal skills development, assistance with managing psychiatric symptoms, as well as, provision of ongoing intensive support to assist clients in resolving potential crisis situations.
• Plan and coordinate functions necessary for discharging clients from the hospital.
• Develop and implement individualized service plans.

• BA in Human Services field or a minimum high school diploma plus 3 years experience in mental health. 
• Experience in social services and behavioral health preferred but not required.
• Position requires frequent driving of own personal vehicle.  A valid driver’s license, vehicle, and insurable driving record are required
