Job Description

Greater Cincinnati Behavioral Health Services (GCB) has an immediate opening for an Outreach Worker within the Clermont County PATH team .This team focuses on helping individuals who are homeless and assesses them for mental health needs.


PATH Outreach Worker

Position Responsibilities:

  • Strives to attain the goals as written in their job performance evaluations and abide by the standards as set forth in GCB’s Compliance Plan.
  • Makes face-to-face contacts with individuals who are homeless throughout Hamilton County to identify potential clients for program (includes making contact with homeless persons on the streets, parks, soup kitchens, shelters, health care facilities, jails, etc.).
  • Assesses individuals who are homeless for mental health needs.
  • Makes multiple contacts as needed and provides basic need items to engage individuals who are homeless and mentally ill.
  • Interacts with agencies, service providers and other community contacts to solicit and receive referrals.
  • Develops psychosocial profile through interviews with individual and information collected from previous service providers.
  • Identifies and connects individuals served to services aimed at addressing mental health issues as well as other basic needs, including medical services and housing.
  • Attends staff meetings and training sessions as assigned.
  • Prepares and submits all required reports of activity, maintain files and records of work activities to provide access to and retrieval of data.  Submits service documentation for outreach contacts by the next working day.
 
Position Requirements:
  • High School diploma required
  • Degree in Social Services and/or two years related experience preferred.
  • Position requires use of personal mobile phone.
  • Position requires frequent driving of the employee’s personal vehicle.
  • Valid driver’s license.
