Scope of Position/Position Summary

This clinical support position serves the Suicide and Trauma Reduction InitiatiVE (STRIVE), one of the Department of Psychiatry amp; Behavioral Healths specialty care programs of distinction focused on suicide and trauma. The Clinical Case Manager facilitates a safe transition to efficient, effective, and compassionate next level of care that ensures patients and research participants are linked to medical and behavioral health services. The Clinical Case Manager monitors the patient care process and physiological needs as well as collects data to evaluate and improve the timeliness and appropriateness of care. The Clinical Case Manager collaborates with physicians, psychologists, social workers, nurses, research staff, and other health care workers to provide comprehensive case management related to treatment, follow-up evaluations, and communication of care plan. The Clinical Case Manager provides services to support the mental and physical health of patients and research participants independently and in collaboration with other members of the team. The Clinical Case Manager maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.

Function

Case Management organizes patient care activities to facilitate the appropriate delivery of health care services and transition of care across the continuum.

Sub-Function

Case Management collaborates with research and health care team members to develop a plan of care for each assigned patient throughout study and/or clinic enrollment. Ensures patient progresses towards desired outcomes by monitoring patient care through assessments and/or evaluations. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.

Minimum Qualifications:

Bachelor's Degree in Social Work, Public Health, Allied Health or other clinically related field. Licensed Social Worker, Licensed Independent Social Worker, Licensed Independent Social Worker - Supervisor, or other advanced license (ex: Respiratory Therapist, Occupational Therapist). 0 years of relevant experience required. 0-2 years of relevant experience preferred.

The Care Management Specialist applies working clinical knowledge and experience within own job discipline and broadens capabilities. Understands business drivers and uses this understanding to accomplish own work. Requires expanded conceptual clinical knowledge in own job discipline and broadens capabilities. Solves problems in straightforward situations; analyzes possible solutions using technical experience and judgements and precedents. Receives a moderate level of guidance and direction. Provides informal guidance to new team members. Explains complex information to others including patients and families in straight forward situations.

Responsibilities include:

  • Receives and reviews referrals to STRIVE and triages incoming referrals to the appropriate research study or care service.
  • Determines eligibility of referred patients and study participants.
  • Obtains informed consent from referred patients and study participants.
  • Identifies patient needs for successful transition to the next level of care.
  • Problem-solves with the patient/caregiver to explore options of care and alternative plans when necessary to achieve desired outcomes.
  • Develops strategies and goals with the patient and/or caregiver(s) to ensure management or resolution of needs and safe transition to the next level of care.
  • Performs ongoing assessment and modifies each patients plan of care based on changes in the client condition lack of response to the care plan patients risk status patient preferences and barriers to care.
  • Reviews the plan of care with the medical team and with the patient and/or caregiver(s) and collaborates with all stakeholders when revisions are needed.
  • Validates plan of care is consistent with clinical pathways and evidence-based guidelines.
  • Facilitates communication and coordination between members of the health care team involving the patient in decision-making as well to minimize fragmentation of services.
  • Coordinates and arranges the patients transition care plan.
  • Refers patients/caregivers to available system and community resources to meet identified needs.
  • Identifies delays in the progression of patient care related to systems issues and collaborates with the multidisciplinary team to remove these barriers.
  • Participates in interdisciplinary team meetings.
  • Assists in the development implementation and monitoring of order sets, clinical pathways, and evidence-based practice.
  • Collects and monitors data on patient care processes and outcomes.
  • Provides services to support the mental and physical health of patients independently and in collaboration with other members of the care team.
  • Coordinates laboratory activities and work assignments of postdoctoral researchers, graduate and undergraduate students, and laboratory research staff.
  • Performs additional duties as assigned.
